I need to file a 4868 tax extension form but I'm confused on where to send it. Can someone help?

You send it to the same address you'd send the tax return to. Most IRS Service Centers are so large that they have their own zip code. All you need is the address as shown in the instructions -- it's just a City, State & Zip, no street address or PO Box number is needed.
